Lumary AH - This article explains how to add NDIS Plan goals to a client's goal attainment plan.
Once you've created a goal attainment plan, you can add agreed service goals to the plan. If your client has NDIS goals, you can use agreed service goals to:
- add extra detail and prioritise
- split large goals into manageable sub-goals
- assign professionals
- document outcomes, and
- add evidence to support goal reviews.
You can also use agreed service goals to monitor a client's progress toward goals beyond their NDIS Plan.

To add a new service goal:
- Navigate to the client's record.
- Click on the Goals tab.
- Click on the New Agreed Service Goal button.
- This will launch the New Agreed Service Goal form.
- The following table describes the fields on this form.
| Field | Purpose |
| Goal Category | Mandatory. Select from the pre-defined list of categories. These may be different depending on your environment. If the category you require is missing, you may need to get a system administrator to add it. |
| Goal Guidance |
Mandatory. This field indicates the guidance to be provided to the user creating the goal, choices being:

- Click Next
- If Create from a Template is selected, and there are Templates created for the selected Goal Category, the following form will be displayed:
- Use the "Search this list..." field to filter the available templates based on their Goal Sub-Category.
- Use the selection field next to the Name of the template to select the template.
- Click Next
- Next you will be prompted to provide the details of the Agreed Service Goal:
- The following table describes the fields on this form:
| Field | Purpose |
| Related NDIS Goal | If the service goal is linked to a specific NDIS goal, you can link them by searching this field for the NDIS goal. |
| Goal Category | Mandatory. Select from the pre-defined list of categories. These may be different depending on your environment. If the category you require is missing, you may need to get a system administrator to add it. |
| Status | Mandatory. The progress of your client toward achieving the goal. When adding goals, this will usually be the default Not Started option. |
| Priority | Mandatory. Higher priority goals will be listed first on the goal attainment plan. |
| Agreed Service Goal | Mandatory. The goal as agreed with the client - If created from a template, this will be pre-populated with the detail on the template. |
| Barriers to Success | Describe any barriers that could prevent or limit the client's success. |
| Actions and Strategies | Describe how the services will help the client achieve the goal - If created from a template, this will be pre-populated with the detail on the template. |
| Professional Responsible | This field allows you to select one or more professionals who will be responsible for supporting the client. Click on an option to select it in the Available list, then click on the arrow to add them to the Chosen list. |
| Hours Required | The approximate number of hours the client will need to achieve the goal - If created from a template, this will be pre-populated with the detail on the template. |
| Timeframe - Est. Number of Weeks | Approximately how many weeks your client will take to reach the goal - If created from a template, this will be pre-populated with the detail on the template. |
| Goal Outcome Review | This is a rich text area for describing the outcome of a goal review. It provides several formatting options, as well as the ability to include images and links to URLs. |
| Evidence to Support Review | This is a rich text are for providing evidence to support a review. It provides several formatting options, as well as the ability to include images and links to URLs. |
- Once you've filled out all of the fields on the form, click on the Save button.
Once you've added service goals, you and your team can monitor the client's progress under the Goals tab on their client record.
