Lumary AH - This article explains how to track a plan review in Lumary AH.
In Lumary AH you can easily track a goal plan review process. This allows you to ensure you complete a review for each plan, providing outcomes of services provided and capturing any recommendations for the next plan period. Once a plan review status is updated to In-Progress additional features become available to capture review details.
This article explains the following function of managing a plan review in Lumary AH:
- updating a plan review status to in-progress
- reviewing goal progress and capturing review reports per goal
- capturing next plan recommendations, and
- updating a plan review status to completed.
Updating a plan review status to in-progress
Updating a plans review status to in-progress enables the following:
- Updates the status for reporting purposes and ensures you maintain compliance.
- Reveals review features on the goal attainment plan.
To place a goal attainment plan review status in progress:
- Navigate to the goal attainment plan you wish to place in progress, you get to the plan easily from:
- the clients profile, on the goals tab
- your clients case list, on the Plan Review column hover.
- The Plan Review Status should highlight on the top of the plan as Not Started.
- Click the Mark Plan Review Status as Complete button, this will update the status to In Progress and highlight the chevron as such.
- This will also make available the Add Recommendation button on the top right of the page, add a tab to the main section for Recommendations and add two fields each of the goals for capturing End of Plan outcome details.
- You are now ready to start capturing the review information.
Reviewing goal progress and capturing review reports per goal
To review goal progress and capture review reports for each goal:
- Use the drop down on each goal and select View.
- On the right of each goal you will see a goal specific progress feed, this will show all case notes related to this goals showing what was worked on and achieved during the plan period.
- Once goal progress has been reviewed, you can then Edit the goal, scroll down the page then capture the formal Goal Outcome Review details and note any Evidence to Support Review.
- You can also update the Goal Status with its achievement status during the plan period.
- Click Save.
Capturing next plan recommendations
Capturing the next plan recommendations are often critical for maintaining or seeing increases to clients funding in order for goal achievement. While the plan review status is in progress you are able to capture and record these recommendations.
- Navage to the goal attainment plan.
- Click Add Recommendation
- A modal will show to capture the recommendation details:
- Discipline - The appropriate discipline related to this recommendation.
- Recommendation Type - Future Goal Setting or Additional Support.
- Recommendation Details - The actual recommendation details.
- Optional:
- Recommended Hours - How many hours are required for this recommendation?
- Recommended Service - What service should the funding requirement for the recommendation be calculated at?
- Rate (if hours and service is select, this is asked for in the next screen)
- Click Next - recommendation will be saved and available under the Recommendations tab on the Goal Attainment Plan.
Updating a plan review status to completed
To indicate a goal attainment plan review is completed:
- Navigate to the goal attainment plan you wish to place in progress, you get to the plan easily from:
- the clients profile, on the goals tab
- your clients case list, on the Plan Review column hover.
- The Plan Review Status should highlight on the top of the plan as In Progress.
- Click the Mark Plan Review Status as Complete button, this will update the status to Completed and highlight the chevron as such.
